A loophole in the Affordable Care Act's Health Insurance Providers Fee appears to benefit payers at the expense of state governments, the Associated Press reported. The fee is meant to pay for the ACA's expansion of coverage, based on the notion that payers can afford it because they gain more customers. To cover the fee, insurers have raised prices for insurers, small businesses and, as it turns out, Medicaid programs.
